All participants hereby confirm that they are voluntarily engaging in an acceptable level of exercise, which has been recommended to me, and that they assume full responsibility for any injury, loss or damages that may result while attending, exercising or participating in any activity hosted or provided by WELLBYNESS HEALTH AWARENESS. In addition, all participants, heirs or legal representatives forever release WELLBYNESS HEALTH AWARENESS and owner(s) from any claim that may arise against WELLBYNESS HEALTH AWARENESS and any of its instructors/consultants from all injuries, claims and damages that  may incur.
